I just installed this app and seems flexible and does just what I needed. It is also priced fairly. Some tweaks were needed to make it work well with my Theme and the Support team were extremely helpful in getting that sorted. I tried three other checkbox apps before choosing this one. It offers the option to have conditional logic to have the checkbox show only for certain products and only certain markets. You can have both conditions (and others) using and/or logic, which gives you great flexibility.
